I assume you meant to say it starts rough and then runs ok. I assume you also meant the first start-up of the day. If that is true, then the cause is usually bad fuel injectors. They are bad because they are old and need to be replaced. Any mechanic can look up your specific engine and tell you if they have that history. That's free. Any mechanic can also give you a free estimate for fixing the problem. If you can't afford that kind of money, then do this: When you first start the vehicle, hold the gas all the way to the floor and turn start for a few seconds. That loads up the fuel rail with gasoline. Now let off the gas and start as you usually do. When the car starts, sit there and hold the gas down a little past idle. Do this for about 15 - 20 seconds. Now you're good to go.

I think what you mean is that it runs rough when you first start it. Depending on what this car is exactly there could be a number of scenarios causing this. Does it have a check engine light on the dash? Is it illuminated? That would be a start.

It means you should have told us the year, make and model of your car so we could compare your problem with other cars like yours or old John Deere A, two cylinder tractors that do make a put,putt,putt,putt exhaust noise.
