Cooling fan relay is broken or fan itself is broken. Try to bypass it direct to 12V (if you think you are capable of doing that). If the fan works wired direct to 12V then your relay is bad. Try replacing that. I would figure that once you got the fan working again it should stay at a cooler temp.

My friend couldn't find the relay for his fan so he wired it off of an ACC circuit in his panel so whenever the car was running the fan was running. Doesn't really hurt anything, aside from the life of the fan motor and the heater being a few degrees cooler in the winter.
