Yes, drain it. Over the years the fuel will have absorbed water via condensation, and that is bad news to to a diesel injection system.......even an old one !
At the very least, I would drain some out of the tank and check for water from condensation. Fresh fuel and new fuel filters would be best.
Do it, to be on the safe side. Wash thoroughly with a solvent or gasoline, let dry. Fill fresh diesel fuel. No additives of any kind, EVER.

I have a 1998 NPR box truck tht has been sitting for years now. I what to try and start it up. Do i have to drain the old fuel? If not what is the recommended process?
