IF it is not supposed to be there, and it is small, a screw might fill the hole. IF it is a drain hole, then leave it alone. Consult your dealer. Regarding the carbon monoxide, if the muffler is at the rear of the vehicle, it will not be problem. However, I have known people who had a mid-car muffler that was bad. After experiencing very bad headaches, the doctor confirmed their poisoning. So, you are wise to ask this question.
As others say, a small, well-formed hole at the bottom of the back of the muffler is a weep hole. Do not plug it up; that can reduce the life of the muffler drastically by letting acidified water collect during short trips.
No generally not a big deal as long as your muffler isn't directly under your cab. I had a hole in mine on my truck for about 6 months and there was no problem besides it was really loud. I later took off the muffler completely and had a glass pac sitting under my truck and for a year I haven't smelled any exhaust in the cab. Over all is it safe? Not really because there's always the possibility when your iddling. Is it really a problem you need to fix? Not really! There's a pretty low chance it'll hurt you especially if your cab is pretty well sealed.
If it is a small perfect hole, the manufacturer out this here for evaporative purposes and it should be left alone so that it can drain water. If it is a rusty imperfect he I would replace the muffler
Most all mufflers have a small drain hole to remove condensation to keep muffler from rusting from inside out. It will not cause carbon monoxide poisoning.
If it is a drain hole (I didn't know they had drain holes myself) but if it is it is alright. If it is a small rust hole it is no problem at all but it will get bigger. Then it will be a noise problem only. But you might as well get a whole new exhaust system if it is rust.

if it,s a small hole then it,s probably a drain hole for moisture and that,s natural for that hole to be there !
